Nurse Practitioner (NP) - Primary Care Summary
- Deliver comprehensive primary care to a geriatric patient population in an outpatient, value-based setting

Duties & Responsibilities
- Conduct thorough patient assessments, physical examinations, and diagnostic evaluations
- Manage chronic diseases such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans tailored to patient goals
- Educate patients and families on medications, nutrition, disease management, and preventive care
- Collaborate regularly with interdisciplinary team members to coordinate comprehensive care
- Prescribe medications and order diagnostic testing within the scope of state practice
- Deliver care through clinic visits, home visits, and telehealth as appropriate
- Maintain accurate documentation and adhere to risk-adjustment and compliance standards

Qualifications & Requirements
- Master’s degree in Nursing required
- Board certification as a Nurse Practitioner (NP) in Adult, Adult-Gero, or Family specialties
- Minimum two years of outpatient primary care experience required
- Ability to practice independently and autonomously
- Experience with value-based care and geriatric populations preferred
- Current, active NP license and DEA (or eligibility)
- Proficiency with electronic medical records; Athena EMR experience highly preferred
- Experience in chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
