The Nurse Practitioner (NP) role is centered on delivering high-quality, patient-centered care in an outpatient setting. The position involves managing a small panel of older adult patients, ensuring continuity of care and individualized treatment plans. Working Monday through Friday, with no nights or weekend shifts, this role provides the ideal balance for professionals seeking both career growth and personal time. Occasional limited call is required, but the first line of triage is handled by a call center, making after-hours contact rare. Nurse Practitioners in this primary care setting benefit from a supportive, interdisciplinary team and access to specialty resources such as endocrinology and podiatry.
As a valued member of the healthcare team, you will conduct patient assessments, perform physical exams, manage chronic conditions such as diabetes, COPD, CHF, and dementia, and develop individualized treatment plans that reflect patient goals. The role also includes patient and family education on medications, nutrition, disease progression, and prevention strategies. The Nurse Practitioner (NP) will coordinate with MAs, schedulers, referral specialists, social workers, pharmacists, and other healthcare professionals to ensure comprehensive care delivery. In addition to clinic-based care, there may be occasional home visits and telehealth appointments to further support patient needs.
The ideal Nurse Practitioner (NP) candidate will have a Master’s degree in Nursing, be board-certified as an Adult, Adult-Gero, or Family NP, and possess a minimum of two years of primary care outpatient experience. Candidates should be comfortable practicing autonomously, and while prior experience with value-based care is preferred, it is not required. Strong EMR skills are essential, and experience with Athena is highly valued. Additional expertise in geriatrics, chronic disease management, wound care, injections, and diagnostic interpretation is a plus. Candidates must hold an active NP license and be eligible for DEA registration.
