The Nurse Practitioner (NP) - Primary Care will find the support of a dedicated care team, including medical assistants, schedulers, referral specialists, social workers, and pharmacists, as well as additional specialty support from endocrinology and podiatry professionals.
In this role, you will be tasked with managing chronic conditions common in the geriatric population, such as diabetes, COPD, CHF, and dementia. You will conduct thorough assessments, develop tailored treatment plans, educate patients and families, and ensure meticulous documentation aligned with risk-adjustment guidelines. The Nurse Practitioner (NP) - Primary Care will also be responsible for prescribing medications within the state scope and delivering care in a variety of settings, including the clinic, patient homes, and via telehealth as needed.

Nurse Practitioner (NP) - Primary Care Summary

Duties & Responsibilities
- Conduct comprehensive patient assessments, including physical exams and diagnostic testing
- Manage and monitor chronic diseases such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans to meet patient goals
- Educate patients and their families about medications, disease progression, preventive care, and nutrition
- Collaborate closely with medical assistants, social workers, pharmacists, and specialty providers
- Prescribe medications within authorized state scope of practice
- Document clinical findings and care plans in the electronic medical record (EMR), with Athena experience preferred
- Deliver care in the clinic, occasionally in patient homes, and via telehealth as necessary
- Participate in a limited call schedule, with most calls initially handled by a call center

Qualifications & Requirements
- Master's degree in Nursing from an accredited institution
- Active, unrestricted Nurse Practitioner (NP) license (state-specific)
- National board certification as an Adult, Adult-Gero, or Family Nurse Practitioner
- Minimum two years of primary care outpatient experience required
- Experience with value-based care models preferred
- Ability to practice autonomously in accordance with state law
- DEA license or eligibility
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation is preferred
- Proficiency with EMR systems; Athena highly preferred
- New graduates are not eligible for this position
