The Nurse Practitioner (NP) – Primary Care will work in an outpatient clinic setting, serving a small patient panel that allows for meaningful patient interaction and continuity of care. The practice emphasizes proactive management of chronic conditions, patient education, and the development of individualized treatment plans. In this role, you will collaborate with an interdisciplinary team including medical assistants, schedulers, referral specialists, social workers, pharmacists, and have specialty support from endocrinology and podiatry experts. In this Nurse Practitioner (NP) – Primary Care position, you will conduct comprehensive patient assessments, perform physical examinations, interpret diagnostic tests, and provide patient education on disease prevention and wellness. The role also involves managing chronic conditions such as diabetes, COPD, CHF, dementia, and debility, while developing and implementing personalized care plans that align with each patient’s individual health goals. - Develop, implement, and monitor individualized treatment plans
- Educate patients and families on medication management, nutrition, disease progression, and preventive health
- Collaborate with interdisciplinary care teams for optimal patient outcomes
- Prescribe medications in accordance with state-specific scope of practice
- Deliver care in clinic, at patient homes, and via telehealth when necessary
- Maintain accurate and thorough documentation in EMR systems, with Athena experience preferred

- Master’s degree in Nursing from an accredited institution
- Current board certification as a Nurse Practitioner (NP) in Adult, Adult-Gero, or Family practice
- Minimum of 2 years’ outpatient primary care experience required
- Experience in value-based care models preferred
- Ability to work autonomously and collaboratively within an interdisciplinary team
- Active state-specific NP license and DEA (or eligibility)
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Familiarity with EMR systems; Athena experience is highly valued
- No new graduates at this time
