- Full-time Nurse Practitioner (NP) role in a physician-led, mission-driven primary care organization focused on seniors.
- Outpatient care only; no night or weekend shifts required.
- Patient population primarily consists of seniors with multiple chronic conditions.

Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, averaging 18 patients per day.
- Deliver comprehensive primary care services with a focus on chronic disease management.
- Work collaboratively with a dedicated care team, including medical assistants and schedulers.
- Participate in care coordination and ensure administrative tasks are completed efficiently during scheduled time.
- Engage with in-house consulting specialists (cardiology, podiatry, etc.) to optimize patient care.
- Promote holistic health solutions, including wellness services such as acupuncture, Tai Chi, yoga, and nutrition.
- Utilize value-based care principles to improve health outcomes and patient satisfaction.
- Maintain accurate, timely medical records and documentation in compliance with organizational standards.

Qualifications & Requirements
