Nurse Practitioner (NP) – Primary Care Summary- Opportunity to practice as a Nurse Practitioner (NP) specializing in primary care, focused on serving seniors with complex chronic conditions
- Collaborate within a multidisciplinary team in a clinician-led setting
- Work in an outpatient environment with an emphasis on value-based, patient-centered care
- Enjoy the benefits of a smaller patient panel, allowing deeper patient relationships
- Be part of an organization committed to quality, cost-effective care and improved health outcomes
- Provide primary care services to a panel of up to 450 patients, predominantly seniors with 3-5 chronic conditions
- Conduct comprehensive patient assessments, develop individualized care plans, and manage chronic and acute illnesses
- See approximately 18-20 patients per day
- Maintain Monday-Friday schedule, with clinic hours from 7:30am to 5:00pm (last patient at 4:15pm); no nights or weekends
- Coordinate closely with an assigned care team, including a medical assistant, scheduler, and referral specialist
- Collaborate with in-house consulting specialists (Cardiology, Podiatry, and more) as needed for patient care
- Integrate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition into patient care plans
- Document care accurately and efficiently, with administrative time built into your schedule
- Participate in quality improvement initiatives and value-based care programs

- Board Certified Nurse Practitioner (NP)
- Minimum of 2 years of outpatient primary care experience as a Nurse Practitioner
- Experience working with geriatric populations is preferred
