As a Nurse Practitioner (NP) - Primary Care, you will be responsible for managing a panel of geriatric patients, typically seeing 18-20 patients per day in a 100% outpatient setting. This role includes covering multiple clinic locations as needed for clinician PTO and patient overflow, enhancing the breadth of your clinical experience. The population primarily consists of seniors managing three to five chronic diseases, and your efforts will contribute directly to the quality and cost outcomes that drive our mission forward.

Nurse Practitioner (NP) - Primary Care Summary- Full-time opportunity focused on outpatient primary care for seniors
- Physician-led, value-based care model with reduced patient panels
- Strong support structure and multidisciplinary clinical resources
- Provide comprehensive primary care to a panel of geriatric patients with multiple chronic conditions
- See 18-20 patients per day during clinic hours (Monday through Friday, 7:30 am – 5:00 pm)
- Work across multiple clinic locations as needed for coverage and patient demand
- Coordinate care with in-house specialists (cardiology, podiatry, etc.) and holistic service providers (acupuncture, nutrition, yoga, tai chi)
- Collaborate with a dedicated care team including a medical assistant and scheduler
- Ensure all care is delivered in alignment with value-based care principles and organizational quality standards
- Participate in administrative tasks during allocated schedule time
- Adhere to a strict outpatient-only schedule with no nights or weekends required
- Competitive salary of $150,000 plus up to $10,000 in bonus (year one, with potential for increase as a partner)

- Board Certified Nurse Practitioner (NP) in Family Medicine or Geriatric Medicine
- Minimum of 2 years’ outpatient primary care experience as a Nurse Practitioner (NP)
- Bilingual Spanish-speaking preferred
- Expertise in managing complex chronic diseases in a geriatric population
