The Nurse Practitioner (NP) – Primary Care plays a key role in providing comprehensive care to a small panel of older adults, focusing on chronic disease management, prevention, and patient education. You will manage a patient volume of approximately 12-16 patients per day, supported by a dedicated care team including medical assistants, schedulers, referral specialists, social workers, and pharmacists. Additional specialty resources such as endocrinology and podiatry are available to enhance patient outcomes. The clinic offers a Monday through Friday schedule, with no nights or weekends, and limited on-call requirements supported by a call center.
As a Nurse Practitioner (NP) – Primary Care, you will conduct patient assessments, develop and implement individualized treatment plans, and provide education to patients and their families. You will collaborate closely with an interdisciplinary team to coordinate care and ensure thorough documentation aligned with risk adjustment requirements. Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Conduct comprehensive patient assessments, physical exams, and diagnostics
- Manage chronic conditions including diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans based on patient goals
- Educate patients and families on medications, nutrition, disease progression, and prevention strategies
- Collaborate with interdisciplinary team members to coordinate and optimize care
- Prescribe medications within state scope of practice
- Provide care in the clinic, patient homes, and via telehealth as needed
- Maintain thorough documentation aligned with risk-adjustment and regulatory standards

Qualifications & Requirements
- Master’s degree in Nursing from an accredited program
- Board certification as a Nurse Practitioner (Adult, Adult-Gero, or Family)
- Minimum of 2 years’ primary care outpatient experience required
- Experience with value-based care and geriatrics preferred
- Ability to practice autonomously
- Active state-specific NP license and DEA (or eligibility)
- Experience in chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Proficiency with Electronic Medical Records (EMR); Athena experience highly preferred
- No new graduates
