As a Nurse Practitioner (NP) – Primary Care, you will be responsible for managing a small panel of patients, typically 12 to 16 per day, allowing you to focus on personalized patient care and chronic disease management. This position emphasizes value-based care, ensuring that the patient experience is central to every interaction. You will be supported by a dedicated care team, including medical assistants, schedulers, referral specialists, social workers, and pharmacists, as well as additional specialty support in endocrinology and podiatry. The interdisciplinary approach ensures that every patient receives the highest standard of care in a coordinated manner.
The work schedule offers an excellent work-life balance, with standard hours from Monday through Friday, 8am to 5pm. There are no night or weekend shifts, and call responsibilities are limited – occurring only a few times per year, with initial triage handled by a call center. This structure allows you to focus on delivering quality clinic-based care, as well as occasional home visits and telehealth consultations when needed. The role requires autonomy, experience, and clinical expertise, particularly in managing older adults with chronic and complex conditions.

Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Conduct thorough patient assessments, physical exams, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans aligned with patient goals
- Educate patients and families on medication, nutrition, disease progression, and prevention strategies
- Collaborate with an interdisciplinary care team to ensure seamless care coordination
- Prescribe medications within the Nurse Practitioner scope of practice
- Deliver care in the clinic setting and, as needed, in patient homes and through telehealth
- Maintain comprehensive and accurate documentation, meeting risk adjustment and regulatory standards

Qualifications & Requirements
- Master’s degree in Nursing from an accredited institution
- Board-certified Nurse Practitioner (Adult, Adult-Gerontology, or Family NP)
- Active NP license (state-specific) and DEA license (or eligibility)
- Minimum of 2 years’ primary care outpatient experience required
- Experience with value-based care models preferred
- Proven ability to work autonomously and make clinical decisions independently
- Experience with geriatrics and chronic disease management preferred
- Familiarity with wound care, injections, ABI testing, or imaging interpretation a plus
- EMR proficiency required; experience with Athena EMR highly preferred
- New graduates are not eligible for this position
