As a Nurse Practitioner (NP) – Primary Care, you will provide individualized healthcare to an older adult patient panel, supporting wellness and chronic condition management. The model emphasizes quality over volume, allowing you to build meaningful relationships with your patients. You will interact closely with a multidisciplinary team including medical assistants, schedulers, referral specialists, social workers, and pharmacists, as well as have access to specialty support in endocrinology and podiatry. The environment provides robust resources to help you deliver evidence-based, patient-centered care, both in the clinic and, as needed, via occasional home visits and telehealth.

This opportunity features a predictable Monday through Friday schedule, free from nights and weekends, which supports work-life balance. Call is limited and primarily triaged by a dedicated call center, ensuring minimal interruptions outside of clinic hours. The small patient panel (12–16 per day) allows for highly personalized care and thorough documentation, essential for effective risk adjustment and outcomes reporting.

As a Nurse Practitioner (NP) – Primary Care, you will conduct comprehensive patient assessments, develop and manage treatment plans tailored to each individual's goals, and provide patient and family education on medications, disease progression, and prevention. You will manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility, utilizing your clinical expertise and experience with geriatric care. Your ability to work autonomously, paired with excellent communication and EMR proficiency, will be critical to your success in this role.
