Nurse Practitioner (NP) – Primary Care Summary- Full-time position serving Medicare patient populations in Chicago, IL – Chinatown
- Collaborative, value-based care model with focus on outcomes and preventive medicine
- Multidisciplinary team support and administrative resources
- Modern clinic with on-site laboratory and EMR systems (Canopy, Greenway)
- Patient-centered approach with community engagement activities
- Provide comprehensive primary care services to a panel of Medicare patients, typically 13-16 per day
- Conduct visits ranging from 20-minute standard appointments to 40-minute diagnostics and new patient evaluations
- Perform regular patient follow-ups every three months or as needed based on complexity
- Collaborate with a care team (including scribe, medical assistant, social worker, and other providers) to coordinate care and manage patient needs
- Utilize value-based care principles to improve patient outcomes and minimize unnecessary hospital admissions
- Document patient encounters thoroughly using Canopy and Greenway EMR systems
- Participate in patient activities and wellness programs offered at the clinic
- Engage with community members to promote preventive health and well-being

- National certification as Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- Bilingual proficiency in Cantonese required
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred)
- Demonstrates a commitment to value-based, patient-centered care
- Enjoys working in diverse, multicultural settings
- Comfortable collaborating within multidisciplinary teams
- Effective communicator with strong documentation and EMR skills
