Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Conduct comprehensive patient assessments, physical exams, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans tailored to patient goals and health needs
- Educate patients and families about medications, nutrition, disease progression, and preventive care strategies
- Collaborate with an interdisciplinary team consisting of medical assistants, schedulers, referral specialists, social workers, pharmacists, and specialty providers such as endocrinologists and podiatrists
- Prescribe medications and therapies within the scope of practice and state regulations
- Provide care in clinic settings, patient homes, and through telehealth platforms as needed
- Maintain complete and accurate documentation aligned with risk-adjustment and regulatory expectations
Salary & Benefits
- Base salary starting from $130,000+, commensurate with experience
- Annual bonus potential up to $10,000

Qualifications & Requirements
- Master’s degree in Nursing from an accredited program
- Active and unrestricted Nurse Practitioner (NP) license, state-specific
- Board certification as an NP (Adult, Adult-Gero, or Family)
- Minimum of 2 years’ experience in outpatient primary care required
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Familiarity with value-based care models strongly preferred
- Ability to practice autonomously; no new graduates considered
- DEA certification or eligibility required
- Proficiency in EMR systems; experience with Athena EMR preferred
