As a Nurse Practitioner (NP) – Primary Care, you will work Monday through Friday, 8am to 5pm, with no nights or weekends required. On-call responsibilities are minimal, occurring just a few times per year, and are primarily managed by a call center. The clinic's small patient panel and strong support staff—including a medical assistant, scheduler, referral specialist, social worker, pharmacist, and access to specialty providers—enable you to focus on delivering comprehensive, high-impact care for older adult patients.

Nurse Practitioner (NP) – Primary Care Summary
- Full-time advanced practice provider role in outpatient value-based primary care
- Focus on older adult/geriatric population

Duties & Responsibilities
- Conduct comprehensive patient assessments, physical exams, and diagnostic evaluations
- Manage and monitor chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized, patient-centered treatment plans aligned with personal health goals
- Educate patients and families on medications, nutrition, disease progression, and preventive care
- Prescribe medications within state scope of practice and ensure safe, evidence-based prescribing
- Collaborate with a diverse care team, including MAs, schedulers, social workers, pharmacists, and specialists (endocrinology, podiatry)
- Provide care in the clinic, at patient homes, and via telehealth as needed
- Maintain accurate, thorough documentation in alignment with risk-adjustment and regulatory standards
- Coordinate referrals and ensure seamless transitions of care

Qualifications & Requirements
- Master’s degree in Nursing from an accredited program
- Board certification as a Nurse Practitioner (NP) – Adult, Adult-Gero, or Family
- Active and unrestricted state-specific NP license
- DEA certification (or eligibility)
- Minimum 2 years of outpatient primary care experience required
- Experience with value-based care models preferred
- Ability to practice independently/autonomously
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Proficiency with electronic medical records (EMR); Athena EMR experience highly preferred
- No new graduates at this time
