We are seeking a dedicated and experienced Nurse Practitioner (NP) to join our dynamic primary care team serving seniors with complex chronic diseases. This full-time role is located in Cleveland, OH, and is designed for those passionate about delivering high-quality, patient-centered healthcare in an outpatient setting. Our clinician-led group is committed to providing coordinated and world-class care to a vulnerable population, with a focus on moderate to low-income seniors who often face multiple chronic health challenges. The practice is 100% outpatient, ensuring no nights or weekends are required, and administrative time is built into your schedule to support work/life balance. The patient panel is intentionally capped at a maximum of 450, allowing for deeper patient engagement and continuity of care. Daily patient volume ranges from 18 to 20, providing the time necessary to address each individual’s unique healthcare needs.
With a full spectrum of in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling, the practice environment supports both traditional and integrative approaches to senior care. Additional patient conveniences, such as door-to-door transportation, further enhance access and continuity of care. Nurse Practitioners (NPs) are also supported in delivering value-based care tailored to geriatric populations, focusing on quality outcomes and patient satisfaction.
