Nurse Practitioner (NP) – Primary Care Summary- Full-time, outpatient Nurse Practitioner (NP) role focused on primary care
- Serve a panel of moderate to low-income seniors with complex, chronic diseases
- Clinician-led culture with an emphasis on teamwork and patient-centered care
- Practice in a modern, value-based care model with holistic health services
- Deliver primary care services to a patient panel consisting primarily of seniors with multiple chronic conditions
- Manage a patient panel of up to 450 individuals, ensuring high-quality, frequent interactions
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with a dedicated support team
- Coordinate with in-house consulting specialists, including cardiology and podiatry, to optimize patient outcomes
- Utilize holistic health services such as acupuncture, Tai Chi, yoga, and nutrition programs as part of integrated patient care
- Maintain accurate and up-to-date patient records within the electronic health system
- Participate in quality improvement initiatives and value-based care programs
- Ensure compliance with all applicable healthcare regulations and organizational protocols
- Engage patients and families in education and shared decision-making to promote long-term health management

- Current and valid Board Certification as a Nurse Practitioner (NP)
- Minimum of two (2) years’ experience as a Nurse Practitioner in an outpatient primary care setting

- Strong clinical assessment, diagnostic, and treatment planning skills
