The role of a Nurse Practitioner (NP) – Primary Care is vital to delivering exceptional healthcare to seniors with complex chronic conditions in a supportive, patient-centered environment. In this full-time position located in Deerfield Beach, FL, the Nurse Practitioner (NP) – Primary Care works closely with a multidisciplinary care team to deliver comprehensive value-based primary care services to moderate to low-income seniors. As a Nurse Practitioner (NP) – Primary Care, you will provide outpatient care in a clinic setting with a focus on geriatric populations, managing 18–20 patients per day. You’ll have the opportunity to practice medicine in a way that prioritizes time spent with each patient, ensuring thorough evaluations and high-quality care. This position offers a structured Monday through Friday schedule with no night or weekend shifts required, promoting a healthy work-life balance. Administrative support, including dedicated medical assistants and schedulers, enables Nurse Practitioners to focus on clinical excellence. Administrative time is also built into the schedule to ensure proper patient care documentation and collaboration with the care team. Nurse Practitioner (NP) – Primary Care: Summary- Full-time outpatient primary care role serving seniors with chronic conditions
- Located in Deerfield Beach, FL, with a patient-centered, value-based care model

- Provide comprehensive primary care to a panel of moderate to low-income senior patients with multiple chronic diseases
- Manage an average of 18–20 patients per day in an outpatient clinic setting
- Participate in value-based care initiatives and collaborate with an integrated care team
- Cover multiple clinic locations as needed for clinician PTO or patient overflow
- Utilize administrative time for patient documentation and care coordination
- Engage in holistic health services, including referrals to in-house specialists (cardiology, podiatry, etc.) and wellness programs (acupuncture, tai chi, yoga, nutrition)
- Foster strong patient-provider relationships through frequent visits and personalized care planning

- Minimum of 2 years of primary care outpatient experience as a Nurse Practitioner
- Bilingual in Spanish preferred but not required
