Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Manage chronic diseases, provide preventive care, and offer palliative care support within assigned facilities
- Coordinate follow-up care and ensure continuity of services for all patients in your panel
- Maintain professional and empathetic communication with patients, family members, and facility staff
- Navigate various facility environments confidently while fostering positive relationships with stakeholders
- Accurately and efficiently document all patient encounters and care activities in the electronic medical record (EMR) system
- Partner with the interdisciplinary team to ensure comprehensive, coordinated, and patient-focused care delivery

Qualifications & Requirements
- Master’s Degree in Nursing (MSN) required
- Active and unrestricted RN and NP licenses in Nevada
- Minimum of one year experience as a Nurse Practitioner; experience in Primary Care, Geriatrics, or Palliative Care preferred
- Strong clinical assessment, diagnostic, and treatment skills
- Excellent communication, interpersonal, and patient relationship-building abilities
