Nurse Practitioner (NP) – Primary Care Summary
- Full-time opportunity focused on Medicare patient populations in a value-based care model.
- Serve a diverse patient base in Chicago, IL, with an emphasis on continuity and preventive care.
- Collaborate with physicians, scribes, MAs, and social workers to provide comprehensive care.
Duties & Responsibilities
- Deliver high-quality primary care services to patients, focusing on prevention, chronic disease management, and health maintenance.
- Conduct thorough assessments including regular, diagnostic, and new patient visits, tailoring care to individual needs.
- Manage an average daily patient volume of 13-16, with scheduled visits every three months or more frequently for complex cases.
- Utilize electronic medical record systems (Canopy and Greenway) for documentation and care planning.
- Collaborate with a multidisciplinary team including physicians, scribes, medical assistants, and social workers to optimize patient outcomes.
- Coordinate care with specialists using resources such as RubiconMD and on-site laboratory facilities.
- Participate in value-based care initiatives to reduce hospital admissions and improve patient satisfaction.
- Engage in community-focused activities and wellness programs offered at the clinic for patient engagement.
- Support transportation and access initiatives for patients within five miles of the clinic.

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
- Active, unrestricted state Nurse Practitioner (NP) license.
- National certification in one or more of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), Gerontological Nurse Practitioner (GNP).
- Active DEA license.
- Bilingual proficiency in Cantonese required to serve local community needs.
- Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred).
