Nurse Practitioner (NP) – Primary Care Summary
- Deliver comprehensive primary care to adult patients in a dynamic outpatient setting.

Duties & Responsibilities
- Provide direct patient care, including preventive health screenings, management of chronic conditions, and acute illness visits.
- Develop and maintain trusting, long-term relationships with patients, ensuring a patient-centered approach to care.
- Monitor patient progress over time, coordinating closely with supervising physicians to optimize care plans.
- Collaborate with interdisciplinary care management teams to support comprehensive healthcare delivery.
- Communicate clearly and compassionately with patients and their families regarding care plans and health goals.
- Thoroughly document patient encounters in the EHR, applying ICD-10, CPT II, and HCC coding for accurate recordkeeping.
- Foster a positive, supportive workplace culture by engaging with colleagues in a team-oriented manner.
- Participate actively in staff meetings, training sessions, and quality improvement initiatives to further the practice’s objectives.

Qualifications & Requirements
- Active and unrestricted Nurse Practitioner (NP) license; recent graduates in the process of licensure are encouraged to apply.
- Experience in primary care, internal medicine, family practice, or related field is valued, but not strictly required.
- Excellent written, oral, and interpersonal communication skills.
