As a Nurse Practitioner (NP) – Primary Care, you will manage a panel of patients, primarily adults and seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension. Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Conduct primary care visits for adult and senior patients (average 10-12 per day)
- Manage a patient panel of up to 650 individuals with complex chronic conditions
- Provide outpatient care, including office-based procedures (suturing, joint injections, skin biopsies, wound care)
- Collaborate with a care team that includes a medical assistant, scheduler, referral specialist, and leadership support
- Participate in value-based care initiatives to improve patient satisfaction, compliance, and health outcomes
- Support patients’ wellness by encouraging participation in clinic activities and health education programs
- Maintain accurate and timely patient records in accordance with clinical standards
- Minimize hospital admissions and readmissions through proactive care management
- Participate in a rotating call schedule as assigned

Qualifications & Requirements
- Current, unrestricted Nurse Practitioner (NP) license
- Minimum of 2 years’ experience in primary care outpatient settings
- Experience with geriatric populations is a plus but not required
- Active DEA registration
- Proficiency in office-based clinical procedures
- Demonstrated commitment to patient-centered, value-based care
- Strong communication and collaboration skills
