Nurse Practitioner (NP) – Primary Care Summary
- Full-time role in a supportive, innovative primary care clinic setting
- Focused on providing comprehensive outpatient care to a senior patient population

Duties & Responsibilities
- Deliver comprehensive outpatient primary care for adult and senior patients, managing chronic and acute health conditions
- Maintain an average daily patient panel of 10-12, with a maximum panel size up to 650 patients
- Apply evidence-based clinical practices to diagnose, treat, and manage multiple complex diseases commonly affecting seniors
- Coordinate care with a dedicated team including medical assistants, schedulers, referral specialists, and clinic leadership
- Conduct office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Participate in a call schedule based on the number of providers in the market
- Contribute to the value-based care model through patient satisfaction, compliance with HEDIS measures, and efforts to reduce hospitalizations
- Promote wellness by supporting activities and resources for patients, including exercise and arts programs
- Document patient encounters and maintain accurate, up-to-date medical records

Qualifications & Requirements
- Current, unrestricted, active Nurse Practitioner (NP) license
- At least 2 years of primary care outpatient experience required
- Experience in geriatrics preferred, but not mandatory
- Current DEA registration
- Proficiency in office-based clinical procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Strong computer and electronic medical record (EMR) skills
- Commitment to value-based care principles and patient-centered service
