Our Nurse Practitioner (NP) role is structured to promote a healthy work-life balance, with a Monday through Friday schedule and no required nights or weekends. The position is fully outpatient and focuses on delivering high-quality, patient-centered care to a small panel of seniors managing multiple chronic conditions. You will have access to a full care team, including dedicated medical assistants and schedulers, as well as in-house consulting specialists such as cardiologists and podiatrists. We also offer holistic health services, including acupuncture, Tai Chi, yoga, and nutrition support, enhancing patient wellness and engagement.

Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Provide primary care services to a panel of up to 450 senior patients, focusing on those with 3-5 chronic diseases
- Conduct comprehensive assessments, develop care plans, and manage ongoing treatment for patients
- Work collaboratively with a multidisciplinary care team, including medical assistants, schedulers, and in-house specialists
- Utilize value-based care models to improve patient outcomes and satisfaction
- Ensure continuity of care through effective communication and follow-up
- Participate in administrative activities, with dedicated time included in your schedule
- Maintain thorough, accurate, and timely documentation in electronic health records
- Promote holistic health services, including acupuncture, Tai Chi, yoga, and nutrition, as part of patient care plans
- Collaborate with the clinic team to provide seamless, door-to-door patient transportation when needed

Qualifications & Requirements
- Board Certification as a Nurse Practitioner (NP) in Family Medicine or Geriatric Medicine
- Current, unrestricted NP licensure
- Experience in primary care, geriatrics, or managing multiple chronic conditions preferred

- Strong communication, organizational, and teamwork skills
