The Nurse Practitioner (NP) will enjoy a predictable Monday through Friday schedule, with no nights or weekends required, and dedicated administrative time built into your workday.

Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients
- See an average of 18 patients per day in a 100% outpatient setting
- Develop, implement, and evaluate patient care plans addressing complex health needs
- Work closely with medical assistants, schedulers, and consulting specialists to coordinate patient care
- Utilize value-based care principles to improve patient health outcomes
- Participate in holistic health initiatives, integrating wellness programs into patient care as appropriate
- Maintain accurate and timely documentation during scheduled administrative time
- Ensure continuity of care by collaborating with other healthcare professionals
- Participate in ongoing quality improvement initiatives within the clinic

Qualifications & Requirements
- Current board certification as a Nurse Practitioner (NP) in Family Medicine or Geriatric Medicine
- State licensure and ability to practice as a Nurse Practitioner (NP)
- Experience providing primary care, preferably to senior populations with chronic conditions
- Strong commitment to value-based care and patient-centered practice
- Excellent communication, collaboration, and organizational skills
- Ability to work effectively in a team-based environment
