Nurse Practitioner (NP) – Primary Care Job Overview
Optigy is seeking a dedicated Nurse Practitioner (NP) to join a value-based care team providing primary care services to Medicare Advantage seniors. This full-time, onsite locums position is located in the Greater Denver Area, CO, and offers an opportunity to make a significant impact on patient well-being within a relationship-centered model of care.
Key Responsibilities
- Conduct comprehensive primary care visits for 12–18 patients per day, focusing on high-quality, relationship-centered healthcare delivery for senior patients.
- Utilize a proprietary electronic medical records (EMR) system designed for optimized senior care documentation and workflow efficiency.
- Collaborate as part of a multidisciplinary care team, including healthcare professionals from various specialties, to ensure holistic and coordinated patient management.
- Emphasize preventive health services, wellness initiatives, and strategies aimed at minimizing unnecessary hospitalizations among the senior population.
- Engage in ongoing communication with patients, families, and the care team to support continuity of care and positive health outcomes.
- Participate in the value-based care (VBC) model, which rewards quality and patient outcomes, not just volume of visits.
- Complete thorough patient assessments, develop individualized care plans, and provide evidence-based clinical interventions tailored to the needs of older adults.
- Adhere to all organizational protocols, compliance standards, and documentation requirements as outlined by Optigy and relevant regulatory agencies.
Qualifications
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license.
- Eligibility for Colorado state licensure is required; an active CO license is strongly preferred.
- Demonstrated experience in geriatrics or value-based care (VBC) is highly desirable.
- Strong clinical assessment, diagnostic, and communication skills, with a commitment to patient-centered care principles.
- Ability to work effectively within a multidisciplinary care team and adapt to evolving care models.
- Familiarity with EMR systems and willingness to learn proprietary platforms specific to senior care.
Position Details
- Employment Type: Full-time Locums
- Location: Greater Denver Area, CO
- Travel, lodging, and malpractice coverage included as part of the assignment package.
- Three-month minimum commitment with potential for conversion to a permanent position based on mutual fit and performance.
