Nurse Practitioner (NP) - Primary Care Job Overview
We are a Federally Qualified Health Center (FQHC) seeking a dedicated Nurse Practitioner (NP) to join our primary care team. This full-time role offers the opportunity to provide comprehensive outpatient care alongside an experienced clinical staff, helping to improve the health and well-being of the local community. As a Nurse Practitioner (NP) in our organization, you will play a key part in delivering high-quality, patient-centered care in a supportive and collaborative environment.
Primary Responsibilities
- Provide primary care services to a diverse patient population, focusing on preventive health, chronic disease management, and acute care needs.
- Collaborate closely with physicians, nurses, and support staff to ensure coordinated and effective patient care.
- Utilize the eClinicalWorks electronic medical record (EMR) system for documentation and patient management.
- Manage a daily caseload of approximately 18–20 patients, maintaining a strong focus on quality and continuity of care.
- Participate in a light phone-only call rotation, typically 3–4 weeks per year, to provide after-hours support as needed.
- Deliver care exclusively in an outpatient setting, with no inpatient or hospital rounding required.
- Engage in continuous quality improvement initiatives and participate in team meetings and professional development activities.
Qualifications
- Current Nurse Practitioner (NP) license and certification, with eligibility to practice in the state as required.
- Demonstrated commitment to providing high-quality, patient-centered care within a primary care setting.
- Strong clinical assessment, diagnostic, and treatment skills, with proficiency in using electronic medical records.
- Ability to work collaboratively in a team-based environment and communicate effectively with patients and colleagues.
- Experience in community health, family medicine, or internal medicine preferred, but not required.
