Our primary care model is designed to offer a small patient panel, allowing Nurse Practitioners to build meaningful relationships with patients and deliver high-quality, coordinated care. As a Nurse Practitioner (NP) - Primary Care, you will manage a panel of senior patients, typically with 3-5 chronic diseases, using evidence-based practices. The role is 100% outpatient, with no nights or weekends required, and includes administrative time within your schedule. You will collaborate with in-house consulting specialists, such as cardiologists and podiatrists, and have access to holistic health services like acupuncture, tai chi, yoga, and nutrition counseling. Door-to-door transportation is provided for patients in need, ensuring their access to care.

Nurse Practitioner (NP) - Primary Care Summary
- Provide primary care to a panel of senior patients in an outpatient setting
- Work with a dedicated support team including your own medical assistant and scheduler
- Focus on chronic disease management, preventive care, and holistic health services
- Collaborate with in-house consulting specialists

Duties & Responsibilities
- Manage a small patient panel (up to 450 patients) with 18 patients seen per day
- Conduct comprehensive assessments, develop care plans, and monitor patient progress
- Coordinate care with consulting specialists such as cardiology and podiatry
- Incorporate holistic health approaches, including acupuncture, tai chi, yoga, and nutrition counseling
- Utilize value-based care models to improve patient outcomes
- Document care activities accurately and efficiently within the electronic health record
- Engage in patient education and health promotion
- Participate in team meetings and collaborate with other healthcare professionals
- Ensure compliance with clinical protocols and regulatory requirements

Qualifications & Requirements
- Current and valid Nurse Practitioner (NP) license
- Board Certification in Family Medicine or Geriatric Medicine
- Experience in primary care or geriatrics preferred

- Proficiency in electronic health records and clinical documentation
