Duties & Responsibilities
- Deliver primary care services to a panel of up to 450 senior patients with 3-5 chronic diseases each.
- Conduct comprehensive patient assessments, develop care plans, and manage ongoing treatment for chronic and acute conditions.
- Collaborate with dedicated support team members, including a Medical Assistant (MA) and Scheduler, to ensure efficient patient flow and high-quality care delivery.
- Provide care in a 100% outpatient setting with no nights or weekends required, promoting an excellent work/life balance.
- Participate in value-based care initiatives, focusing on improved patient outcomes and preventive health strategies.
- Utilize administrative time built into the schedule to complete documentation, coordinate care, and participate in team meetings.
- Refer patients to in-house consulting specialists such as cardiology and podiatry as needed for comprehensive care.
- Engage patients in holistic health services, including acupuncture, tai chi, yoga, and nutrition counseling, to support overall well-being.

Qualifications & Requirements
- Board Certification in Family Medicine or Geriatric Medicine required.
- Active and unrestricted Nurse Practitioner (NP) license in the relevant state.
- Demonstrated experience working with senior populations or patients with complex healthcare needs preferred.

- Strong clinical, communication, and organizational skills.
