Nurse Practitioner (NP) / Physician Assistant (PA) - Primary Care Summary- Full-time role in a reputable family medicine and primary care clinic
- Serve adult and senior patient populations with comprehensive care
- Contribute to a high-volume, well-established outpatient clinic
- Work collaboratively with experienced healthcare professionals
- Focus on preventive medicine, chronic disease management, and patient education
- Conduct thorough patient assessments including medical history, physical examinations, and diagnostic evaluations
- Develop and implement patient-centered treatment plans, utilizing evidence-based guidelines
- Manage acute and chronic health conditions for adults and seniors in an outpatient setting
- Order and interpret diagnostic tests such as labs and imaging studies
- Prescribe medications and provide ongoing medication management as needed
- Educate patients and families on health promotion, disease prevention, and management strategies
- Document all patient encounters and clinical findings accurately in the electronic health record (EHR)
- Collaborate with physicians, nurses, and ancillary care providers to ensure coordinated care delivery
- Maintain current knowledge of primary care trends, best practices, and regulatory requirements

- Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
- Certification through an accredited national board (AANP, ANCC, NCCPA, or equivalent)
