Nurse Practitioner (NP) / Physician Assistant (PA) – Family Medicine Summary
- Serve as a key member of a multidisciplinary team delivering outpatient family medicine services
- Focus on adult and senior patient populations, managing acute and chronic health conditions

Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Perform routine wellness visits, preventative screenings, and chronic disease management
- Diagnose and treat common acute illnesses and injuries
- Order and interpret diagnostic tests and imaging as needed to support clinical decision-making
- Prescribe medications and other treatments within the scope of practice and regulatory guidelines
- Educate patients and families about healthy lifestyle choices, disease prevention, and medication adherence
- Collaborate closely with physicians, nurses, and allied health professionals to optimize patient outcomes
- Document all patient encounters and care plans accurately and in compliance with clinic policies

Qualifications & Requirements
- Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) licensure
- Graduate of an accredited NP or PA program
- Board certification in family medicine, primary care, or relevant specialty preferred
- Experience in an outpatient family medicine or primary care setting is highly desirable
