We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient concierge medicine practice in Bradenton, FL. As a key member of our collaborative healthcare team, you will provide comprehensive care to adult and geriatric patients, focusing on high-quality, patient-centered outcomes in a supportive and community-oriented environment. Responsibilities
- Deliver comprehensive primary care services to adult and geriatric patients within an outpatient setting.
- Conduct thorough physical examinations, assess patient histories, and accurately diagnose acute and chronic conditions.
- Develop, implement, and manage individualized treatment plans, including medication management, preventive care, and chronic disease management.
- Utilize electronic medical records (EMR) systems, specifically eClinicalWorks, to document and maintain accurate patient information.
- Collaborate closely with a multidisciplinary healthcare team to promote optimal patient outcomes and continuity of care.
- Participate in value-based care initiatives, focusing on quality metrics and patient satisfaction.
- Provide patient education and counseling, empowering individuals to take an active role in their health and wellness.
- Optional participation in on-call rotations (no required call duties).
- Maintain compliance with all applicable regulatory and organizational standards.
Qualifications
- Current board certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Active Florida license or eligibility to obtain licensure in the state of Florida.
- Demonstrated knowledge or experience in value-based care models preferred.
- Strong commitment to patient-centered care and the ability to work effectively within a collaborative team environment.
- Excellent communication, organizational, and clinical decision-making skills.
