Responsibilities- Conduct comprehensive patient examinations and review medical records to assess overall health.
- Order and interpret diagnostic tests such as blood tests, X-rays, and other relevant procedures to support clinical decision-making.
- Diagnose a variety of health conditions and illnesses, providing evidence-based care to diverse patient populations.
- Develop and propose treatment plans for chronic diseases, ensuring continuity of care and optimal patient outcomes.
- Prescribe medications in accordance with current guidelines and within the scope of licensure.
- Maintain accurate and up-to-date patient records, ensuring compliance with all regulatory requirements and best practices in documentation.
- Educate patients and family members regarding health conditions, prescribed medications, disease prevention, and healthy lifestyle choices.
- Promote preventive care and good health practices within the community and among patients.
- Stay current with advancements in medical knowledge and primary care practices through ongoing continuing education and professional development.
- Collaborate effectively with physicians, nursing staff, and other healthcare professionals to deliver coordinated, patient-centered care.
- Valid and active Illinois state license as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Current DEA registration for prescription authority.
- Proven experience working as a Nurse Practitioner or Physician Assistant in a primary care or outpatient clinic setting.
- Proficiency with Electronic Medical Records (EMR) systems is required.
- Comprehensive knowledge of healthcare regulations and standards related to primary, acute, and chronic care.
- Excellent communication skills, strong ethical standards, and a demonstrated ability to work collaboratively as part of a multidisciplinary team.
- Strong patient-centered approach, with compassion, patience, and the ability to build trust with individuals and families.
- Commitment to ongoing professional growth and willingness to participate in continuing education opportunities.
