Primary Care Nurse Practitioner (NP) or Physician Assistant (PA-C) - Summary- Deliver patient-centered primary care services as part of a multidisciplinary clinical team
- Utilize advanced clinical knowledge to address acute and chronic health conditions
- Foster collaborative relationships with physicians, nurses, and support staff
- Promote wellness, preventive care, and patient safety
- Contribute to a supportive, inclusive, and forward-thinking workplace culture
- Provide comprehensive primary care services, including assessment, diagnosis, and treatment of acute and chronic conditions
- Promote preventive health and wellness through patient education and routine screenings
- Collaborate with physicians and other clinical professionals to develop and implement care plans
- Perform in-office procedures and manage diagnostic testing as required
- Prescribe medications and monitor patient response to therapy
- Participate in quality initiatives and continuous team development
- Respond to emergencies and ensure adherence to patient safety protocols
- Maintain strict confidentiality in accordance with HIPAA guidelines and organizational policies

- Minimum of 2 years of experience in primary care or related clinical setting
- Active, unencumbered license as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
- Current DEA registration
- Active Basic Life Support (BLS) certification
- Commitment to maintaining continuing education as per scope of practice and licensure requirements
- Demonstrated ability to deliver high-quality, evidence-based patient care
- Experienced clinician with a strong background in primary care
- Exceptional communication, collaboration, and organizational skills
