We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our healthcare team at Swiftcreek Family Care. This full-time position offers a dynamic opportunity for qualified advanced practice providers to make a significant impact on patient care and outcomes. As a valued member of our clinical staff, you will collaborate with physicians and multidisciplinary team members to provide exceptional medical services to a diverse patient population.
Key Responsibilities
- Facilitate new patient consultations, conducting thorough intakes to assess patient history, current symptoms, and reason for visit.
- Perform comprehensive physical examinations and screenings, utilizing evidence-based protocols and clinical judgment.
- Order and interpret diagnostic tests, including laboratory work such as blood tests and imaging studies, to assist in the evaluation of patient conditions.
- Analyze test results and clinical findings to diagnose health problems, develop differential diagnoses, and establish appropriate treatment plans.
- Recommend individualized treatment options, outline future appointment needs, and provide ongoing patient education to support health and wellness.
- Coordinate with other healthcare professionals by providing referrals to specialists or ancillary services as clinically indicated.
- Deliver clear after-care and ongoing care information to patients and their families, ensuring understanding of follow-up instructions and available resources.
- Maintain meticulous and up-to-date electronic health records for all patients, documenting test results, immunizations, changes in condition, and care plans in compliance with regulatory standards.
Qualifications
- Current and valid licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) as required by state and federal regulations.
- Completion of an accredited Nurse Practitioner or Physician Assistant program.
- Demonstrated clinical competence in family practice or primary care settings.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate effectively with colleagues.
- Strong organizational and documentation abilities to ensure accurate recordkeeping and compliance with healthcare standards.
