Nurse Practitioner (NP) – Inpatient Transplant Summary
- Deliver advanced, direct patient care in an inpatient transplant unit, supporting patients before and after organ transplantation.
- Collaborate with physicians, surgeons, pharmacists, social workers, and other health team members to develop and implement patient care plans.
- Participate in complex case management and support quality improvement initiatives within the transplant service line.
- Educate patients and families about transplant procedures, post-operative care, medication management, and lifestyle modifications.
Duties & Responsibilities
- Perform comprehensive assessments of transplant patients, including medical histories, physical examinations, and review of diagnostic findings.
- Order, interpret, and follow up on laboratory and imaging studies to guide clinical decision-making.
- Develop, implement, and modify individualized care plans in collaboration with multidisciplinary teams.
- Monitor patient progress, manage complications, and provide evidence-based interventions as needed.
- Educate patients, families, and caregivers on transplant processes, health maintenance, and post-discharge care.
- Document assessments, treatment plans, and outcomes accurately and in a timely manner in the electronic health record system.
- Participate in hospital rounds, case conferences, and quality improvement initiatives relevant to transplant care.
- Maintain current knowledge of transplant medicine, clinical guidelines, and best practices.

Qualifications & Requirements
- Current and valid Nurse Practitioner (NP) license.
- Master’s or Doctoral degree in Nursing from an accredited institution.
- Board certification as a Nurse Practitioner (NP) in a relevant specialty (e.g., Adult, Family, Acute Care).
- Preferred experience in inpatient care, critical care, or transplant medicine.
- Ability to obtain and maintain hospital privileges as required.
