Nurse Practitioner (NP) – Home Health Summary
- Deliver transitional post-discharge care for patients in a home health setting.
- Collaborate closely with medical directors and interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.

Duties & Responsibilities
- Conduct comprehensive post-discharge visits, including in-depth medication reconciliation and assessment of patient needs.
- Evaluate and address social service requirements, ensuring patients have access to necessary community resources and support services.
- Provide ongoing follow-up care to monitor patient recovery, identify potential complications, and adjust care plans as needed.
- Coordinate care with primary care providers, specialists, and social workers to facilitate smooth transitions and minimize hospital readmissions.
- Manage a daily caseload of 6-7 patients, balancing efficiency with personalized attention to each individual’s unique needs.
- Document all care activities accurately and in compliance with organizational and regulatory standards.
- Utilize evidence-based clinical guidelines to deliver high-quality care in alignment with organizational best practices.
- Engage in regular communication with the supervising Medical Director and other team members to review complex cases and receive guidance as appropriate.

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.
- National certification as a Nurse Practitioner in at least one of the following specialties: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, non-probationary state Nurse Practitioner license.
- Current DEA license for prescriptive authority.
- Authorization to work in the United States.
