Nurse Practitioner (NP) - Home Health Summary- Deliver primary care services directly in patients’ homes
- Provide comprehensive assessments and consultative care for Medicare-aged individuals
- Collaborate with multidisciplinary teams to ensure continuity and quality of care
- Participate in quality improvement and care planning initiatives
- Serve a diverse and often underserved patient population
- Conduct in-home primary care visits, assessments, and consultations
- Partner with leadership to schedule patients for home visits
- Collaborate with Medical Assistants and care teams to facilitate optimal patient outcomes
- Maintain accurate, timely documentation of all services and patient encounters
- Participate in ongoing quality improvement activities
- Provide care within an assigned geographic area, utilizing reliable transportation
- Ensure patient satisfaction through empathetic communication and clinical excellence
- Competitive base salary with total potential compensation exceeding $143,000

- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Missouri
- BLS/ACLS certification required
- Background in geriatrics, internal medicine, or family medicine preferred
- Experience managing patients with multiple chronic conditions
- Ability to lift and transport up to 20 lbs. of medical equipment
- Strong clinical judgment, communication, and empathy skills
- Reliable vehicle and valid driver’s license required
