Nurse Practitioner (NP) – Home Health Job Overview
Optigy is seeking a dedicated and compassionate Nurse Practitioner (NP) to join a leading value-based care organization, delivering high-quality home-based primary care services for a Medicare-aged population in Kansas City, MO. This full-time position focuses on improving access to healthcare and health outcomes for underserved communities, particularly seniors, by providing direct care in patients’ homes.
Key Responsibilities
- Provide comprehensive primary care and consultative services to patients in their residences within the assigned Kansas City, MO area.
- Work closely with leadership to identify and schedule home visits for patients in need of medical attention.
- Collaborate with Medical Assistants and multidisciplinary care teams to ensure continuity of care, optimal patient satisfaction, and adherence to care plans.
- Maintain accurate and timely documentation of all patient encounters and medical services provided, in compliance with organizational and regulatory standards.
- Participate in ongoing quality improvement activities and contribute to the development and execution of individualized care plans.
- Ensure reliable transportation is used to travel within the designated service area, bringing necessary medical equipment to patient homes as required.
Qualifications
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Missouri.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Preferred experience in geriatrics, internal medicine, or family medicine settings.
- Demonstrated ability to manage patients with multiple chronic conditions, providing comprehensive care and effective management strategies.
- Physical ability to lift and transport up to 20 lbs. of essential medical equipment to patient homes.
- Excellent communication, empathy, and clinical decision-making skills to foster strong patient-provider relationships and deliver high-quality care.
- Reliable vehicle and a valid driver’s license required for travel throughout Kansas City, MO and surrounding areas.
