Nurse Practitioner (NP) – Home Health Summary- Deliver home-based primary care to Medicare-aged patients in Las Vegas, NV
- Work with a value-based care team committed to quality and patient satisfaction
- Serve underserved and at-risk populations, providing essential healthcare services directly in their homes
- Engage in a dynamic, supportive, and flexible work environment
- Provide primary care and consultative services in patients’ homes
- Partner with leadership to identify and schedule patients in need of home visits
- Collaborate with Medical Assistants and interdisciplinary care teams to ensure continuity and quality of care
- Maintain accurate and timely documentation of all patient services and clinical encounters
- Participate in ongoing care planning and quality improvement initiatives
- Deliver care within an assigned geographic area, utilizing reliable transportation
- Competitive base salary with total earning potential exceeding $140,000

- Current BLS/ACLS certification

- Strong clinical decision-making, communication, and empathy skills
- Ability to lift and transport up to 20 lbs. of equipment as needed
- Reliable vehicle and valid driver’s license required
