Nurse Practitioner (NP) – Home Health Summary
- Deliver home-based primary care to Medicare-aged patients in Las Vegas, NV

Duties & Responsibilities
- Provide primary care and consultative services in patients’ homes
- Partner with leadership to identify and schedule patients in need of home visits
- Collaborate with Medical Assistants and interdisciplinary care teams to ensure continuity and quality of care
- Maintain accurate and timely documentation of all patient services and clinical encounters
- Participate in ongoing care planning and quality improvement initiatives
- Deliver care within an assigned geographic area, utilizing reliable transportation

Qualifications & Requirements
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license
- Current BLS/ACLS certification
- Experience in geriatrics, internal medicine, or family medicine preferred
- Proven ability to manage patients with multiple chronic conditions
- Strong clinical decision-making, communication, and empathy skills
- Ability to lift and transport up to 20 lbs. of equipment as needed
- Reliable vehicle and valid driver’s license required
