Nurse Practitioner (NP) – Home Health Job Description
Optigy, in partnership with a value-based care organization, is seeking a dedicated Nurse Practitioner (NP) to deliver high-quality, home-based primary care services. This full-time opportunity is based in Las Vegas, NV, and focuses on providing comprehensive healthcare to Medicare-aged individuals in the comfort of their own homes. The Nurse Practitioner (NP) – Home Health role is crucial in enhancing access to care and optimizing health outcomes for underserved seniors.
Responsibilities
- Deliver primary care and consultative services directly in patients’ homes, focusing on preventative care, chronic disease management, and wellness education.
- Partner with clinical leadership to identify, schedule, and prioritize patients who will benefit from in-home visits in the Las Vegas, NV area.
- Collaborate closely with Medical Assistants, care coordinators, and interdisciplinary teams to ensure quality care, continuity, and optimal patient satisfaction.
- Maintain accurate, timely documentation of all patient encounters and services provided, adhering to regulatory and company standards.
- Participate actively in ongoing quality improvement initiatives and contribute to comprehensive care planning for the patient population.
- Deliver care within an assigned geographic area, requiring reliable transportation to reach patient homes in Las Vegas, NV.
Qualifications
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Nevada.
- BLS/ACLS certifications required.
- Experience in geriatrics, internal medicine, or family medicine preferred.
- Demonstrated ability to manage patients with multiple chronic conditions and complex care needs.
- Physical ability to lift and transport up to 20 lbs. of equipment as needed for patient visits.
- Strong communication, empathy, and clinical decision-making skills essential for effective patient care.
- Reliable vehicle and valid driver’s license required to fulfill full-time field-based responsibilities in Las Vegas, NV.
