The Nurse Practitioner (NP) – Home Health is supported by a dedicated care team including a Medical Assistant, Scribe, and Social Worker, as well as direct mentorship from a Medical Director. Your daily responsibilities will reflect the same depth and rigor as in a clinical setting, with an average of six to seven home visits per day. This structure ensures comprehensive, coordinated, and patient-focused care, allowing you to focus on outcomes rather than volume.

- Deliver personalized primary healthcare services to older adults in their homes as part of a value-based care model.
- Work closely with a multidisciplinary team dedicated to optimizing patient wellbeing and independence.

Duties & Responsibilities
- Conduct comprehensive clinical workups and assessments during home visits, providing the same level of care as in traditional clinical settings.
- Manage an average of six to seven patient visits per day, focusing on quality outcomes and preventative care.
- Collaborate with a dedicated care team comprised of a Medical Assistant, Scribe, and Social Worker to deliver integrated care solutions.
- Maintain thorough and accurate patient documentation and care plans.
- Work under the guidance of a Medical Director who provides mentorship, clinical guidance, and professional development support.
- Advocate for patient-centered approaches and contribute to ongoing quality improvement initiatives.
- Participate in care coordination, follow-up, and communication with patients, families, and other care providers as needed.
Salary & Benefits
- Competitive base salary with a range of $105,000 - $135,000, plus performance-based bonuses.
- Experienced providers with bonuses may earn $150,000 - $160,000 annually.
- Annual clinical performance bonuses: $15,000 for NPs with less than 5 years of experience, and $25,000 for those with 5+ years.
- Mileage and toll fee reimbursement provided for all job-related travel.
Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from a Nurse Practitioner Certificate Program.
- National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, non-probationary state Nurse Practitioner license required.
- Active DEA license required.
- Authorization to work in the US.
