Nurse Practitioner (NP) – Home Health About the Role
This full-time Nurse Practitioner (NP) – Home Health position offers the opportunity to join a value-based care group dedicated to delivering personalized primary care to older adults. As a Nurse Practitioner, you will be an integral part of a collaborative care team, providing comprehensive home-based assessments and primary care services to a diverse geriatric patient population.
Responsibilities
- Conduct an average of 6-7 home health visits per day, performing thorough clinical assessments, diagnosis, and management of chronic and acute conditions for Medicare patients.
- Deliver patient-centered care in coordination with a dedicated care team, including a Medical Assistant, Scribe, and designated Social Worker, to ensure optimal patient outcomes.
- Partner closely with the Medical Director, receiving ongoing mentorship and guidance to support clinical practice and professional growth.
- Complete comprehensive workups and documentation during each patient visit, mirroring the standards of clinical practice found in office-based settings.
- Participate in care planning, follow-up, and quality improvement initiatives designed to enhance the patient experience and promote health maintenance.
- Utilize a value-based approach that prioritizes quality over volume, actively engaging patients in their care plans and helping them achieve their health goals.
- Collaborate with interdisciplinary team members to address the social, behavioral, and medical needs of patients, ensuring holistic and coordinated care.
Qualifications
- Graduate of a Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or Nurse Practitioner Certificate Program.
- National board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, unrestricted Nurse Practitioner (NP) license in good standing.
- Active DEA license, with authorization to prescribe medications as required by state and federal regulations.
- Authorization to work in the United States.
