As a Nurse Practitioner (NP) – Home Health, you will play a crucial role in providing quality post-discharge care, ensuring patients experience smooth transitions from hospital to home and supporting their ongoing well-being.

As a Nurse Practitioner (NP) – Home Health in Rockford, IL, you will contribute directly to our mission of keeping patients healthy and helping them live their lives to the fullest.
Key Responsibilities- Conduct transitional health assessments and post-discharge visits for patients in their homes throughout Rockford, IL.
- Perform medication reconciliation to ensure patients understand and adhere to their prescribed regimens.
- Coordinate social service follow-ups and provide referrals to community resources as needed.
- Deliver general follow-up care, monitoring patient progress and identifying any early signs of complications.
- Manage a daily caseload of approximately 6-7 patients, focusing on individualized care plans that address each patient's unique needs.
- Work collaboratively with the Medical Director and other healthcare professionals to ensure continuity and quality of care.
- Document all patient interactions and care activities in accordance with company policies and regulatory requirements.
- Graduate of a Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or an accredited Nurse Practitioner Certificate Program.
- Hold current national certification in one or more of the following specialties:
- Family Nurse Practitioner (FNP)
- Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
- Adult Nurse Practitioner (ANP)
- Gerontological Nurse Practitioner (GNP)
- Possess an active, non-probationary Nurse Practitioner license in Illinois.
- Hold an active DEA license for prescriptive authority.
- Authorization to work in the United States.
