Nurse Practitioner (NP) – Home Health Summary- Full-time Nurse Practitioner (NP) specializing in home health and transitional post-care.
- Serve patients in Rockford, IL, focusing on post-discharge and transitional care services.
- Integral member of a patient-centered, mission-driven healthcare organization.
- Opportunity to support older adults in maintaining health and independence at home.
- Perform comprehensive post-discharge visits in patients' homes, including health assessments and care planning.
- Conduct medication reconciliation to ensure safe and effective medication management.
- Coordinate follow-up care, liaise with social services, and facilitate access to community resources.
- Manage a daily caseload of 6-7 patients, ensuring timely and thorough care delivery.
- Collaborate with the Medical Director and interdisciplinary team for clinical support and case discussions.
- Document patient interactions, care plans, and outcomes in accordance with organizational and regulatory standards.
- Advocate for patients' needs and help reduce hospital readmissions through proactive care management.
- Competitive salary range: $105,000 - $135,000, with potential to earn up to $160,000 including performance bonuses.
- Annual clinical performance bonuses based on experience:
- Less than 5 years’ experience: $15,000 bonus
- 5+ years’ experience: $25,000 bonus

- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
- National board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Nurse Practitioner (ANP), or Gerontological Nurse Practitioner (GNP).
- Active, unrestricted state Nurse Practitioner license.
- Active DEA license.
- Authorization to work in the United States.
- Demonstrated commitment to providing high-quality, patient-centered care in home health or transitional settings.
- Strong communication and interpersonal skills for effective collaboration with patients, families, and interdisciplinary teams.
- Detail-oriented, organized, and adept at managing multiple priorities in a dynamic environment.
- Proficient in clinical documentation and electronic health records.
- Ability to work independently with support from a Medical Director as needed.
