Nurse Practitioner (NP) / Advanced Practice Provider (APP) – Summary- Full-time Nurse Practitioner (NP) / Advanced Practice Provider (APP) position in Tucson, AZ.
- Focus on value-based healthcare for Medicare patient populations.

- Provide comprehensive, equitable, and effective medical care to a panel of Medicare patients.
- Conduct regular patient visits (average 13-16 per day), including routine, diagnostic, and new patient assessments.
- Develop and implement individualized care plans in collaboration with a multidisciplinary team.
- Utilize on-site laboratory services and access to specialist consultations through RubiconMD.
- Document patient encounters using EMR systems (Canopy and Greenway).
- Participate in preventive medicine initiatives and patient wellness activities.
- Ensure accurate documentation, including HCC, ICD-10 coding, and Health Risk Assessments where applicable.
- Engage in ongoing communication with patients, families, and care team members to ensure optimal outcomes.
- Support community engagement through patient activities such as exercise classes, wellness events, and educational programs.

- Active, non-probationary Nurse Practitioner (NP) license in the state of Arizona.
- Active DEA license required.
- Bilingual proficiency in Spanish is preferred.
