Nurse Practitioner (NP) – Full-Time
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our value-based healthcare team, where the focus is on delivering high-quality care to Medicare patient populations. As a Nurse Practitioner, you will work collaboratively within a multidisciplinary care team to provide exceptional patient-centered services. Our model is structured to reward positive patient outcomes, preventive medicine, and an outstanding healthcare experience. This is an excellent opportunity for a Nurse Practitioner looking to make a difference in a supportive, innovative environment.
As a Nurse Practitioner (NP) with us, you will be an integral part of a team that includes scribes, medical assistants, and social workers, all working together to achieve the best outcomes for our patients. You will benefit from access to on-site lab facilities, a supportive work culture, and specialized tools such as RubiconMD for specialist consultations. Our clinics are equipped to provide a wide range of services and activities for patients, including transportation within a five-mile radius and an active community room with wellness programs. You will see an average of 13-16 patients per day, with appointments tailored to patient complexity, ensuring enough time for comprehensive care and relationship-building. Our electronic medical records (EMR) systems, including Canopy and Greenway, are designed to streamline your workflow and enhance documentation accuracy.
Our value-based care approach aligns incentives with reduced hospital admissions, enhanced preventive care, and improved patient satisfaction. The multidisciplinary care team structure allows you to focus on quality over quantity, fostering a rewarding work environment. As a Nurse Practitioner (NP), you will have opportunities for professional development, including a partnership track for physicians, generous paid time off, continuing medical education stipends, and tuition reimbursement. Comprehensive benefits include health, vision, dental, and life insurance, medical malpractice coverage, and a robust retirement plan with immediate vesting and company match. We also offer flexibility in sign-on options, making it easier to transition into our team.
Qualifications for the Nurse Practitioner (NP) role include a graduate degree in nursing (MSN, DNP, or NP certificate), national certification in family, adult, adult-gerontology, or gerontological nursing, and active licensure as a Nurse Practitioner. Experience with HCC documentation, ICD-10 coding, and health risk assessments is preferred, as is proficiency in Spanish.
