Nurse Practitioner (NP) - Summary- Full-time Nurse Practitioner (NP) position in a physician-led primary care organization
- Focus on delivering value-based, patient-centered care to seniors with chronic conditions
- Outpatient-only schedule with no nights or weekends
- Collaborative, mission-driven team environment
- Provide comprehensive primary care to a panel of senior patients, typically 13-16 patients per day
- Develop, implement, and manage individualized care plans for patients with multiple chronic diseases
- Work closely with a dedicated support and administrative team to ensure exceptional patient outcomes
- Participate in value-based care initiatives and quality improvement activities
- Allocate scheduled administrative time for charting and care coordination
- Maintain high standards of professionalism, empathy, and respect in all patient interactions
