Nurse Practitioner (NP) - Summary
- Deliver primary care services in a value-based outpatient setting.
- Care for a small patient panel of older adults, focusing on quality outcomes.
- Collaborate with a dedicated multidisciplinary team including an MA, scheduler, referral specialist, social worker, and pharmacist.

Duties & Responsibilities
- Conduct comprehensive patient assessments, physical exams, and diagnostic evaluations for geriatric patients.
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility.
- Develop individualized treatment plans that align with patient goals and value-based care principles.
- Educate patients and their families about medications, nutrition, disease progression, and preventive health strategies.
- Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
- Prescribe medications in accordance with state regulations and clinical best practices.
- Provide care in multiple settings, including the clinic, patient homes, and via telehealth as needed.
- Maintain accurate, thorough clinical documentation in alignment with risk-adjustment expectations and regulatory requirements.
- Participate in limited call rotation, with primary triage handled by a call center to minimize after-hours interruptions.
- Engage in regular communication with specialty support providers, including endocrinology and podiatry, to enhance patient care.

Qualifications & Requirements
- Master’s degree in Nursing from an accredited program.
- Board certification as a Nurse Practitioner (NP) in Adult, Adult-Gerontology, or Family specialties.
- Minimum of 2 years of primary care outpatient experience required.
- Demonstrated experience providing care to geriatric patients and managing chronic diseases.
- Strong ability to practice autonomously and collaboratively within a team-based model.
- Active NP license in the state of Georgia (or eligibility to obtain).
- DEA certification (or eligibility to obtain).
- Experience with value-based care models is preferred.
- Expertise in wound care, injections, ABIs, or imaging interpretation is a plus.
- Proficiency with EMR systems; experience with Athena is highly preferred.
- No new graduates; prior experience is essential.
