Duties & Responsibilities
- Deliver primary care support to patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD), ensuring that their complex health needs are addressed through comprehensive assessments and care planning.
- Manage high-risk, medically complex patients, focusing on reducing readmissions and supporting transitions of care to facilitate optimal health outcomes.
- Collaborate closely with dialysis teams, nephrologists, and other healthcare professionals to coordinate interdisciplinary care and ensure continuity for each patient.
- Utilize both in-person and telehealth modalities to monitor patient progress, manage symptoms, and adjust care plans as needed.
- Build and maintain meaningful, ongoing relationships with patients and their families to support their long-term health and well-being.
- Engage in continuous quality improvement initiatives and participate in case conferences to enhance patient care strategies.
- Document all patient encounters, care plans, and progress notes accurately and in compliance with healthcare regulations and organizational standards.

Qualifications & Requirements
- Active, unrestricted Nurse Practitioner (NP) license in the state of California, with current Board Certification.
