Nurse Practitioner (NP) – Summary
- Full-time advanced practice provider delivering value-based care to Medicare patients
- Collaborative, team-based clinical environment in Lancaster, PA
- Focus on preventive medicine and quality outcomes over service volume
- Access to on-site lab, specialist consultation, and patient transportation resources
Duties & Responsibilities
- Provide comprehensive primary care to a panel of adult and geriatric patients on a consistent schedule
- Conduct routine and complex health assessments, including 20-minute regular visits and 40-minute diagnostic or new patient appointments
- Maintain a patient visit volume of 13-16 patients per day
- Utilize EMR systems (Canopy and Greenway) to document care and ensure compliance with value-based care metrics
- Collaborate with a multidisciplinary team including scribes, medical assistants, and social workers
- Coordinate care transitions, health risk assessments, and chronic disease management
- Engage patients through community room activities and supportive services such as transportation and wellness programs
- Participate in quality improvement initiatives and contribute to organizational goals for patient outcomes

Qualifications & Requirements
- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner certificate program
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
- Active, unrestricted Nurse Practitioner license
- Active DEA license
- Experience with Hierarchical Condition Categories (HCC), ICD-10 coding, and Health Risk Assessments preferred
- Bilingual Spanish-speaking skills are highly valued
