As a Nurse Practitioner (NP), you will be responsible for providing comprehensive primary care, with a focus on managing chronic conditions and promoting overall wellness. Working within a value-based outpatient model, you will collaborate with a dedicated interdisciplinary care team to ensure optimal patient outcomes. Your role will include performing assessments, developing individualized care plans, educating patients and their families, and utilizing telehealth and home visit services as needed. The position emphasizes continuity of care, proactive management, and a holistic approach to the needs of the geriatric population.

Nurse Practitioner (NP) - Summary
- Provide comprehensive primary care to an older adult population in a value-based outpatient setting
- Collaborate with an interdisciplinary care team to coordinate holistic patient care
- Manage a small patient panel for personalized, high-quality care
- Utilize clinic-based, telehealth, and occasional home visit modalities
- Work Monday through Friday, with limited call responsibilities
Duties & Responsibilities
- Conduct patient assessments, physical exams, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans based on patient goals and clinical guidelines
- Educate patients and families on medication regimens, nutrition, disease progression, and preventive care
- Prescribe medications within the scope of practice and state regulations
- Collaborate with care team members (medical assistant, scheduler, referral specialist, social worker, pharmacist) to ensure coordinated care
- Document encounters thoroughly and accurately to support risk adjustment and compliance
- Participate in telehealth visits and occasional home visits as needed
- Utilize electronic medical records (EMR), with preference for Athena experience

Qualifications & Requirements
- Master's degree in Nursing from an accredited program
- Current, active NP license (state-specific required)
- Board certification as an Adult, Adult-Gero, or Family Nurse Practitioner
- Minimum of 2 years of primary care outpatient experience
- Experience with value-based care models preferred
- Ability to practice autonomously and make informed clinical decisions
- Experience managing geriatric patients and chronic diseases, including wound care, injections, ABIs, or imaging interpretation (preferred)
- DEA license or eligibility required
- Proficiency with electronic medical records (EMR); Athena experience highly preferred
- New graduates are not eligible for this position
