Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct comprehensive patient assessments, physical exams, and diagnostic evaluations in an outpatient clinic
- Manage chronic conditions, such as diabetes, COPD, CHF, dementia, and debility, through evidence-based treatment plans
- Develop and implement individualized care plans that align with patient goals and preferences
- Educate patients and families on medications, nutrition, disease progression, and preventive health strategies
- Collaborate closely with a multidisciplinary care team, including medical assistants, schedulers, referral specialists, social workers, and pharmacists
- Coordinate care with specialists, including endocrinology and podiatry as needed
- Prescribe medications in accordance with state regulations and practice standards
- Provide patient care through multiple modalities: in-clinic, at-home visits, and telehealth support as appropriate
- Maintain thorough and accurate documentation consistent with risk-adjustment and value-based care expectations
- Participate in limited on-call responsibilities, with primary triage handled by a call center

Qualifications & Requirements
- Master’s degree in Nursing from an accredited institution
- Current board certification as a Nurse Practitioner (NP) – Adult, Adult-Gero, or Family
- Minimum of 2 years of outpatient primary care experience
- Demonstrated ability to practice autonomously
- Active state-specific Nurse Practitioner (NP) license
- DEA license or eligibility
- Experience with value-based care models preferred
- Experience managing geriatric patients and chronic disease conditions preferred
- Proficiency in electronic medical record (EMR) systems; Athena experience highly preferred
- Experience in wound care, injections, ankle-brachial index (ABI) measurements, or imaging interpretation is a plus
- New graduates are not eligible for this position
