Nurse Practitioner (NP) - Summary- Full-time role focused on value-based healthcare for Medicare patient populations
- Supported by a multidisciplinary care team to optimize patient outcomes
- Emphasis on preventive care, low hospitalization rates, and patient engagement
- Located in Rock Hill, SC, a thriving and welcoming community
- Deliver high-quality, patient-centered care to Medicare populations in a value-based care setting
- Engage in regular patient visits, averaging 13-16 patients per day
- Perform comprehensive new patient assessments and ongoing diagnostic visits (20-40 minutes per appointment)
- Collaborate with a care team including scribes, medical assistants, and social workers
- Utilize onsite laboratory services and access specialist consultations via RubiconMD
- Maintain accurate electronic medical records using Canopy and Greenway systems
- Ensure compliance with best practices in clinical documentation, HCC, ICD-10 coding, and health risk assessments
- Participate in community wellness activities and patient engagement initiatives
- Coordinate patient care transitions and follow up as needed

- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner (NP) license
- Active DEA license
- Familiarity with HCC documentation, ICD-10 Coding, and Health Risk Assessments preferred
- Experienced Nurse Practitioner (NP) committed to value-based, patient-focused care
- Strong collaborative mindset and ability to thrive in a multidisciplinary team
- Skilled in EMR systems and clinical documentation
