Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Provide primary and preventive care to Medicare patients as a Nurse Practitioner (NP)
- Manage a patient panel with an average volume of 13-16 patients per day
- Conduct regular 20-minute visits and comprehensive 40-minute assessments for new or complex patients
- Collaborate with a multidisciplinary team, including physicians, scribes, medical assistants, and social workers
- Utilize on-site laboratory services and telehealth access to specialists
- Document patient care using Canopy and Greenway EMR platforms
- Participate in value-based care initiatives to reduce hospital admissions and improve preventive health outcomes
- Engage in community activities and patient wellness programs held within the clinic
- Ensure compliance with regulatory requirements and best practices in clinical documentation, including HCC, ICD-10, and HRA elements
- Support patient transportation needs within a 5-mile radius of the clinic

Qualifications & Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or graduation from an accredited Nurse Practitioner (NP) certificate program
- National certification in one of the following: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner (NP) license
- Active DEA license
- Experience or understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
- Bilingual Spanish speaking skills are a plus
