Nurse Practitioner (NP) - Summary
- Deliver value-based primary care to Medicare patients as a Nurse Practitioner (NP).
- Collaborate closely with a multidisciplinary team including scribes, medical assistants, social workers, and primary care physicians.
- Utilize advanced EMR systems (Canopy and Greenway) and onsite diagnostic labs.
- Provide patient visits focused on preventive care and chronic disease management.
- Engage in patient education, health risk assessments, and care coordination.
Duties & Responsibilities
- Conduct comprehensive assessments, diagnostic visits, and routine follow-ups for a diverse Medicare patient population.
- Manage an average daily patient volume of 13-16 individuals, with visit lengths tailored to patient needs.
- Document all patient interactions and care plans accurately using EMR systems.
- Work collaboratively within a value-based care model to enhance patient outcomes and satisfaction.
- Participate in care team meetings, case reviews, and community health activities.
- Access specialist consults via platforms such as RubiconMD as needed.
- Promote and facilitate patient engagement activities, including exercise, wellness events, and educational sessions.
- Ensure compliance with regulatory requirements and best practice protocols.

Qualifications & Requirements
- Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited NP Certificate Program.
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active and unrestricted state Nurse Practitioner license.
- Current DEA license.
- Familiarity with HCC documentation, ICD-10 coding, and health risk assessments preferred.
- Bilingual Spanish-speaking ability is a plus.
