Nurse Practitioner (NP) - Summary- Deliver value-based primary care to Medicare populations in a team-focused clinical setting.
- Work collaboratively with multidisciplinary teams, including physicians, scribes, medical assistants, and social workers.
- Utilize advanced EMR systems and on-site diagnostic resources to enhance patient care delivery.
- Contribute to community health through patient education, wellness programs, and preventive care initiatives.
- Conduct comprehensive health assessments, regular follow-up visits, and full diagnostic evaluations as a Nurse Practitioner (NP).
- Manage a patient panel with an average daily volume of 13-16 patients, allowing for individualized care.
- Document all encounters accurately using Canopy and Greenway EMR systems.
- Lead and participate in preventive medicine programs, chronic disease management, and health risk assessments.
- Work in conjunction with care teams to coordinate patient care and reduce avoidable hospitalizations.

- Current DEA license.
