Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct comprehensive health assessments, regular follow-up visits, and full diagnostic evaluations as a Nurse Practitioner (NP).
- Manage a patient panel with an average daily volume of 13-16 patients, allowing for individualized care.
- Document all encounters accurately using Canopy and Greenway EMR systems.
- Lead and participate in preventive medicine programs, chronic disease management, and health risk assessments.
- Work in conjunction with care teams to coordinate patient care and reduce avoidable hospitalizations.
- Utilize on-site labs, phlebotomy services, and telehealth specialist consultations for comprehensive care delivery.
- Support patient engagement in community room activities such as exercise, wellness events, and educational workshops.

Qualifications & Requirements
- Graduate of an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner (NP) Certificate Program.
- Current national certification in one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner (NP) license.
- Current DEA license.
- Experience or familiarity with HCC documentation, ICD-10 coding, and health risk assessments is preferred.
- Bilingual skills in Spanish are highly valued due to patient population needs.
